IMPORTANT:  Things to do and forms to return in order to complete your child’s registration:

Fill out, sign and return the following forms:

1.  Student Registration form  (Blue)
2.  Parent Contract  (Blue)
3.  PTA Activity Form (Purple)
4. Student Application (Prek - 8th) ONE PER CHILD (White)
5.  Parent Service Agreement (availability form) (Yellow)
6. Individual Pupil Request for Loan of Textbooks Form – ONE PER CHILD (Salmon)
7. B6T Application for Private School Transportation Form – ONE PER CHILD (Pink)

**These forms are not included in the PK packet therefore, do not require return.

You must use the BLACKBAUD Tuition online enrollment form to sign up for a monthly payment plan, unless paying in full for the year.

You must provide copies of your child’s immunization records, Birth certificate, and Baptismal certificate, (as well as any other sacraments received).

2. $85 PTA Fee – One per family
